Your practice provides a list of patients that could benefit from the program and we handle the rest. Alternatively, our team can also help identify eligible patients and reach out to complete enrollment.
Our team of experienced care managers reaches out to patients, informs them that their primary care provider has referred them to the program, and obtains consent to finalize enrollment.
We create behavioral health care plans, facilitate and coordinate treatment such as psychotherapy & counseling, and provide continuity of care with a designated member of the care team.
Patients receive support for both physical and mental health through their primary care provider, reducing the need for multiple appointments or referrals.
Behavioral health symptoms like anxiety or depression can be identified sooner, helping patients manage issues before they become more serious.
Patients work with a dedicated care manager who collaborates with their primary care provider and behavioral health specialists to create a care plan tailored to their unique needs.
Receiving mental health support in a familiar primary care setting helps reduce stigma and makes it easier for patients to talk openly about emotional or behavioral concerns.
General Behavioral Health Integration (BHI) Care Management services, at least 20 minutes of clinical staff time directed by a physician or other qualified healthcare professional, per calendar month, for managing patients with behavioral health conditions. Includes assessment or follow-up monitoring, including use of validated rating scale(s), behavioral health care planning, including revisions, treatment coordination, and continuity of care with a designated member of the care team.
