Program Requirements

Patient Eligibility

Patients who have at least 1 chronic condition that require regular monitoring qualify for RPM. These conditions are typically diabetes and hypertension.

Patient Consent & Time Requirements

Our team obtains patient consent before initiating monitoring. We inform patients about the nature of the monitoring, the data being collected, and how it will be used to manage their care. We also make sure to meet CMS requirements for both readings (16) and time spent with patients each month (20 minutes).

Care Plan

Our team works with your provider to develop a care plan that ensures that the monitoring is medically necessary, aligns with the patient's treatment goals, and provides clear instructions for how the RPM data will be used to adjust or optimize care.
