Remote Patient Monitoring (RPM) is a healthcare solution that allows medical professionals to monitor patients' health outside of traditional clinical settings. Through the use of wearable devices and digital tools, healthcare providers can track vital signs such as blood pressure, heart rate, glucose levels, and more, in real-time. RPM helps ensure continuous care, allowing for early detection of potential health issues and more personalized treatment plans. This innovative approach reduces hospital visits, improves patient outcomes, and increases convenience for both patients and providers.
Patients who have at least 1 chronic condition that require regular monitoring qualify for RPM. These conditions are typically diabetes and hypertension.
Our team obtains patient consent before initiating monitoring. We inform patients about the nature of the monitoring, the data being collected, and how it will be used to manage their care. We also make sure to meet CMS requirements for both readings (16) and time spent with patients each month (20 minutes).
Our team works with your provider to develop a care plan that ensures that the monitoring is medically necessary, aligns with the patient's treatment goals, and provides clear instructions for how the RPM data will be used to adjust or optimize care.
Patients receive regular check-ins, medication reviews, and assistance navigating their care plan—ensuring nothing slips through the cracks.
Early intervention and consistent monitoring help identify issues before they escalate, reducing avoidable hospital stays and emergency visits.
Each patient’s care is tailored to their unique needs and updated regularly, with our care team reaching out to offer timely support and education.
Patients no longer feel alone in managing their health—they have a dedicated team they can rely on, fostering trust and better self-management.
